I took my insert out and replaced it with a bottle cap (I was bored). There is a notch (that I didn't know about until later) to the left side of the insert. I think if you use a flat pocket screw driver, it should pop right out. Like everyone else, I boogered up the shifter knob a bit trying to remove it.

I thought about another shifter knob, but I kinda like the (cheesy) bottle cap look. :D

i have also read here that if you use a heat gun or hair dryer to warm up the shift knob, it comes out easier.

Thanks for the tip. I'm going to try and remove mine to reposition it as it is crooked now. Bugs me.

 
The correct way to remove both the 5 and 6 speed shifter knobs is to twist it off. Nothing is destroyed.

Most of the time you will not be able to budge it. There is a nut inside that should be removed first. IF you can just twist it off, fine, but that is not the correct way to do it.
 
I will not disagree, they are very tight... but if you do budge it whether its with some sort of grip, or your manliness, how can it not be the correct way, there is no damage done?
