Once you shear the OEM ones off, just use a regular bolt.
Search for "bolt fish wire" and use that to run the bolt in from the top down instead of trying to get the nut to stay up top.

Sounds like an ignition tumbler issue. I'd take some of the dash apart and hotwire it (jumper the terminals) & see if it turns over.

I'd bet it's a little of both. A clogged cat will have a rattle to it that you can almost feel in the motor. An italian tune up can temporarily change the behavior of a failing cat, but not an O2.
