I used the upper hole for the taillight as the guide and then self tapping bolts for the rest.

Had to be mindful of the edge closest to the gate so it didn't rub the paint off, but...

I used the same lines on my 99 and had the same concern. I took some old heater hose tubing, cut a split into it, and wrapped it around the brake line where it made contact and zip tied it in place...

Standard configuration. You have to remove the stud on each side so you can use a longer bolt with the spacer. PITA to remove the stud. Others have worked around it. Use the search feature.
