RPM Drop in Reverse and Sudden Decel

I have a '95 GC Laredo with 210k.

When I'm going slowly in reverse, or rapidly decelerating (i.e. stopping at a light, etc..) the RPM's drop to around 2 or 3 hundred and the Jeep shakes a might. I'm going to delve into it this weekend, any ideas of what it might be? Thanks.
