need help with problem effecting transmission

for quite some time now (months :O ) my transmission is not catching until 40, or 70mph, anything in between it revs way up to 4000+ until it shifts, always at those two speeds. but at 40 and 70 it runs at normal rpms and seems fine, when it first started i took it in to have transmission fluid replaced..they told me it was just fine, ive tried stop slip, did not effect anything. and every so often when i come to a stop sign, and then reapply the gas the car keeps lurching forward as if it was running out of gas, violent jerking forward. the check engine light has been on the whole time too :S any suggestions? could be multiple problems...i have no idea, i have no knowledge what so ever about cars thanks for your help, any is appreciate

do a "key dance" to check codes. I just solved a problem with shifting on my rig and it was the the TPS sensor. the sensor was not bad but the connecting the pigtail was making to the plug was bad. At idle if I tapped or wiggled on my TPS pigtail it would cause the car to studder and even die, While driving my rig would randomly refuse to shift out of first. so check your TPS and the connection. but first do a "key dance" and narrow down the problem.
