Bad ball joint or not?

I've got a 6" skyjacker lift and recently had to replace some bushing in the front end. After the work, it's been squeaking more than before. So I looked underneath to see if I need to add some grease and noticed something odd with a ball joint.

On one of the ends, the ball side/end had a shim (or something that looked like that) move off center. It looks like it should be in line with the rod. Without a pic it's difficult to explain. I'm trying to find out if this needs to be replaced or can just be fixed/moved into the correct position etc. Would that be something that I can do myself (I have very few tools)?
