Stock 79 would be a Dana 20 (I'm assuming you meant you didn't know the model, not what the transfer case is). Any of the major vendors will have rebuild kits for them. It's also a good time to consider some upgrades: TeraLow and/or TwinStiks.

Rebuilds are simple, if you have access to a shop press, even simpler. You will need a dial indicator and magnetic base to set the end play. Other than setting the end play, the most difficult part I found was tightening the nuts on the yokes. Make yourself a spanner wrench:

And have a 'big friend' help. Or a small friend and use an impact.

Since you didn't tell us anything other than '79, it would be hard to tell exactly what transfer case you have...

If you have manual trans, you probably have a Dana 20 transfer case.
If you have an automatic trans, you probably have a Borg Warner 1339 'Quadra-Trac' transfer case...

If you have a manual trans, and the transfer case shifter arm is FLAT stock, it's almost always going to be a Dana 20.

If you have a ROUND transfer case shifter handle, it might be a Dana 300.

IF you Hi/Low shifter is on the transmission tunnel hump just in front of the drivers seat, and the transfer case switch is in the glove box, you have a Borg Warner 1339.

You REALLY need to give details of engine, transmission, ect. before asking questions so we can give viable answers.
