T150 won't shift after clutch replacement

The tranny shifted fine before I drove it to break in the new clutch (I tried a few light clutch engagements - both foward and reverse - before actually driving it.) It shifted OK at first, but shortly, I could only get the tranny to shift into 2nd.
I replaced the clutch disc, pressure plate, clutch release bearing, clutch release fork, pilot bushing, clutch linkage rod, and bell crank. I repaired the worn ends of the clutch rod (pedal arm to bell crank). The clutch will engage/disengage properly in the only gear I can currently get it to shift into (2nd).
I took it all apart because it seemed like the clutch was worn out, but i found the the alignment pins for the throw-out bearing on the clutch fork had worn off, allowing one side of the throw-out bearing to slip off. The clutch disc was hardly worn at all, but the fingers on the pressure plate, and the throw-out bearing both had odd wear patterns

Any ideas?

Sounds like the clutch is not disengaging. With the engine off will it slip into all gears? If so it is probably a question of adjustment.

x2, adjust that linkage and you should be good to go.
