It mounts to the eight factory bumper crossmember bolts. If you choose to add the unibody tie in brackets, than it mounts to the underside of the uniframe where the hitch mounts.

Ideally yes. However some aftermarket hitches have some crappy threaded blocks that are a PITA to work with.
