Need help connecting header to exhaust

I am having some trouble figuring out how my header is to be connected to the supplied reducer.

The reducer supplied with my Hedman Hedder is similar to these http://www.summitracing.com/parts/HED-21110/?rtype=10

My question is, how do I use it attach the header to the rest of the exhaust system?
Are the reducer flange and fiber gasket sandwiched between the 3 bolt flange and header flange?
Is the three bolt flange supposed to be welded to the reducer? If so, should the reducer pass through the flange? And on which side is it welded- the gasket side (I wouldn’t think this would seal very well) or the outlet side?
Does this setup require an exhaust ‘donut’ (none was provided)?

1. Slip the reducer through the metal flange
2. Reducer is welded to the exhaust pipe
3. Slip fiber gasket fits between the header flange and exhaust flange
4. Bolt them together.
