Transmission Slips

Question about my Auto Transmission on my 98 ZJ 4X4 (with NP242 xfer case). When I first cold start the car and put in gear it is like the torque converter is slipping. Takes a few seconds until it locks up and the car is moving correctly. Also notice sometimes when it shifts it goes loose between gears (revs shoot up, then it engages next gear). This happens only sometimes and can when it is shifting itself or I am manually shifting. Vehicle has about 190k miles.
Any ideas on the issue?
Is there a better trans that can be easily swapped in (maybe with more gears) or is this one the one to have?

Have you checked the trans fluid level, dropped the pan and replaced the filter, add new fluid? How's the transfer case's fluid level. When was the last time it was changed?

Fluid level is fine and it looks good in color. It has been a little while since changed (filter & fluid). I was thinking that might help.
