If you don replace all mounts at once the new ones will deteriorate in a cupel of weeks to were it would be recommended to replace them again

It is the motor or transmission mounts. Mine does the same thing.

I have the vibration issue too, and it only got worse after adding the lift. I noticed when I put the transfer case in neutral, the vibration seem to go away. Do you think this could still be an issue with the mounts, or an issue with the t-case?

First thing would be to check the mounts.
It could be the mounts, cuz when U put the TC in neutral ,I don't think there would be a load in the engine.
Maybe some one else will chime in with more detail

do you have to have an engine crane to change the mounts? Or how do you go about supporting the engine while changing them?

Jack the engine, With a 2 by, board across the oil pan.I used short pc. of 2X8
