Noise in the transmission

Hi, I have a 93 YJ, AX5 Manual 5 speed transmission standard, my problem is this:

When turning on the jeep not hear any noise in the transmission, but after a while I start to hear a noise like metal to metal and when I put decelerated fourth gear speed is thrown, I took off the transmission and changed one bearing then stopped the noise for a couple of days and was no longer thrown fourth gear, I noticed the gears were great.

After a couple of days I noticed a leak and had to go back down the transmission because it was missing an o ring and was leaking clutch oil pump and when put it back continued to be hearing the noise and again the fourth gear thrown away. Now I feel less powerful speeds.

Anyone have any idea what is happening?. Anyone know why the noise?

By the way, when i push the clutch stop the noise

Pretty sure that's a bad throw out bearing. Someone correct me if I'm wrong.
