Slipping transmission or a bad mount ???

Ok, so here are the symptoms :

1) When I step on it hard from a stand still, the car seems to "clunk" into gear, then it's fine.

2) When I stop sometimes, it seems to sometimes "clunk" when I come to stop.

3) It mostly seems to do this when warmed up.

Any thoughts?

Clean and grease the slip yoke at the transfer case. Use the Google, there are several threads on the topic.
